Community Health Teams

Taking care of your health can feel overwhelming, especially when you’re not sure where to start. That’s where the Community Health Teams (CHTs) come in! CHTs is a service offered by Nova Scotia Health and the IWK to adults living in the greater Halifax area, including Fairview–Clayton Park. CHTs offer free group programs and one-on-one wellness navigation with the goal of helping residents make small, manageable changes that support long-term well-being.

What Kind of Programs Are Being Offered?

  • Healthy eating and nutrition
  • Mental wellness and stress management
  • Physical activity
  • Parenting support
  • Managing chronic conditions and risk factors

The programs are open to anyone aged 18+ and have a valid Nova Scotia health. No referrals are required to join the CHTs programs. Sessions are available in person or virtually through Zoom for Healthcare for easier access to those with limited mobility, transportation or time to participate.

Where To Start?

If you are unsure which programs are right for you, or are facing barriers to health like food insecurity, housing concerns, medication or access to services, the CHT’s Wellness Navigation service can help. This free service connects residents to community resources, healthcare supports, and local services. Navigation appointments are available by phone, in-person and Zoom. You must have a valid Nova Scotia health card.

These programs are inclusive and can provide accessibility support (including translation services).

Services for Families, Children and Youth

Families can also access support through IWK’s wellness navigation team. If you’re concerned about your child’s mental health, looking for youth programming, or navigating supports like housing or medication coverage, staff are available to help guide the way.
Additional Supports for the Black Community
The Nova Scotia Brotherhood and Nova Scotia Sisterhood offer free, culturally relevant healthcare to Black men and women in the province. Their teams provide primary care, wellness navigation, health education, and support managing chronic conditions.
